The Metropolitan Palliative and Community Care Service (MPaCCS) is seeking a skilled and highly motivated Social Worker to join our team. This role is vital to delivering high-quality, holistic end-of-life and palliative care services. The successful applicant will primarily serve the South and Southeast metropolitan areas of Perth, though work in the Northern metropolitan area may be required at times.
About the role
This is a part-time position, offering 60-75 hours per fortnight. In this role, you will educate and mentor residential care facility staff to enhance their capacity to provide quality palliative care. Your responsibilities will include:
- Supporting and mentoring staff delivering palliative care
- Facilitating advance care planning, case conferencing, and shared decision-making
- Communicating effectively at the end of life
- Delivering culturally safe care, including LGBTI
- Providing access to end-of-life support services and resources
